Why is the liver checked separately in diabetes?

In diabetes texts the liver often sits on the culprit side: the organ releasing sugar into the blood at night, deaf to insulin. It has a second role that comes up less. Once fat builds up, it too becomes an organ that gets checked. This second role has its own measures. The paper’s enzyme line is only one.
The liver carries two roles at once in this picture, and they mix easily. In the first it’s a source: the release pushing up the morning reading starts there. In the second it’s a target. Fat fills in among its own cells, the tissue stiffens in time, and the organ moves to the result side, not the cause. “What is the link between weight and diabetes?” sees the same organ from the cause side.
In most people the built-up fat stays put. The reference text compiling liver diseases calls plain fatty buildup a picture that can reverse and doesn’t progress; progression shows up in the part where inflammation is added on top. There the tissue stiffens, and the picture at the far end of the stiffening is called cirrhosis. The picture has no way of announcing itself either: most patients feel nothing. Its name on the paper changed recently. The old name was fatty liver disease; today it goes by a name putting the metabolic origin up front.
The prevalence number is read on this ground. The American Diabetes Association’s consensus report writes that roughly two in three people living with type 2 diabetes have fat in the liver. The same report also names a gap outright: liver health has never entered the front row of followed complications like eye, kidney and nerve. The liver carries its own line on the medication side too: “How many groups do diabetes medications fall into?” splits groups by the organ they touch.

All four measures go quiet somewhere different. The paper’s enzyme line isn’t enough alone: the same reference text writes that mildly raised enzymes are the most common finding, yet in most patients they run normal. A normal line doesn’t clear the organ. Indexes figured from blood were developed for this gap; the most reachable is called FIB-4, wants no new test, and is figured from numbers already on the paper. Its power gathers at the ends: direction sharpens at the low and high end, accuracy drops in the middle band, even more so at older ages and in type 2 diabetes. In the method that measures stiffness from outside, the reading loses reliability as body measurements grow. The tissue sample is the method that settles the diagnosis, kept for cases whose result stays uncertain.
What’s left: the two measures don’t answer the same question from the same place. The enzyme line reports the cell’s state that day. The index doesn’t read the same line alone; it counts it alongside age and another number in the blood panel. So the index can rise while the enzyme line looks ordinary. On the liver side these two lines are read separately.
