What are ketones, and when are they checked?

Ketones are the backup fuel the liver makes from fat when cells can’t use glucose well enough. A small amount is ordinary; the problem starts once they build up. This article covers what ketones are, why blood and urine show different things, and which situations bring the subject up.
Cells draw energy mostly from glucose, and need insulin to bring that fuel inside. When insulin’s effect falls short, glucose piles up in the blood, yet can’t get inside the cell. The body then turns to its second fuel. The liver starts breaking down fatty acids, and out come compounds called ketones. These are real fuel for muscle and brain, so ketones alone don’t count as a disorder.
After a long hungry day, or a long walk, a small amount of ketones forming is ordinary. The picture starts to change at the point where production outpaces use. Once ketones build up, the blood’s acid balance is upset, and the shift gives its own outward signs. The main ones: nausea, belly pain, deep and fast breathing, a fruity smell in the mouth.
Buildup speeds up when insulin’s effect falls below the body’s need that day. Sometimes that effect vanishes altogether. More often, insulin sits there but can’t do the job. Guidelines gather both under one heading: the shortfall is absolute or relative, both ending at the same door. Once buildup starts, the picture develops within hours, not over days. So ketones carry information separate from the sugar itself.
Ketones are checked two ways, and the two don’t give the same information.

The difference isn’t a small detail. The urine strip reports late, because urine carries what was filtered a few hours ago. As the picture starts to clear, the strip can still give a dark color, because the kind it catches briefly rises during that clearing. Blood measurement, though, shows the moment you’re in; that’s why guidelines put it first.
Ketones don’t come up every day, and this isn’t a subject needing constant follow-up. It comes forward in certain situations:
- Sick days coming in between: when infection signs like fever, chills, burning when passing urine or diarrhea strain the body
- Vomiting, or fluids not staying down
- Insulin somehow falling short, say a blocked pump set
- A reading staying high longer than expected
- One of the signs above joining the picture
The result’s size and what comes with it gain meaning together. Waiting out in the open on a damp bathroom shelf can throw off a strip’s color; the box’s expiration date affects the result too. A thermometer measures fever without naming the cause; a ketone measurement likewise describes the situation. What the result means, the care team decides by looking at the whole person.
There’s one exception. If vomiting, belly pain, deepening breathing or drowsiness sits beside the reading, the picture doesn’t wait for the next appointment. A separate article lists which signs don’t wait.
